Short Term Accommodation (STA)
Here at Productive care support we provide assistance with
accommodation and tenancy obligations as support to guide, prompt or undertake activities to
ensure that participant obtains/retains appropriate accommodation.
Daily Activities
Productive care support provides support for people with a
disability to maintain their independence within their home we assist people with a disability to
undertake and or develop skills to maintain
Social & Community Participation
At Productive care support we provide flexible opportunities
for people to engage in their community and enjoy a range of purposeful and recreational
activities that are tailored to each person’s goals and aspiration.
Household Tasks
At Productive care support staff assists and undertakes with domestic
chores this includes tasks such as, cleaning stoves, wiping down bench tops and inside fridges.
As well as vacuuming or mopping floors, dusting, dishwashing, changing bed linen, washing,
drying, along with doing essential ironing.
Group/Centre activities
Reachability Centre Based Activities and Groups promote
socialization while offering both worker facilitated and self-directed capacity building activities.
Participants are welcome to undertake activities independently within the space, participate in
facilitated workshops or a combination of both.
Supported Independent Living (SIL)
This program is designed to empower individuals with disabilities by providing support tailored to their unique needs. Through SIL, individuals receive personalized assistance to live independently while addressing various aspects of their daily lives, including housing, personal care, and community engagement.
Development- Life skills
At Productive care support we assist with personal activities that
assist you to live as independently and comfortably as possible. Our qualified, passionate, as
well as dedicated staff are well trained, friendly and will support you in ensuring you achieve
your daily living goals.
Plan Management
Our Qualified plan management will help you monitor your funds and
provide financial reporting. At productive care support we will give you the freedom and control
to use your NDIS budget with the help of a plan management.
Assistive Prod-Household Tasks
assistive products for household tasks are specialized
aids and equipment that will assist you in the home environment. Our services Productive care
support provide assistive devices for household tasks.
Assist Prod-Pers Care/Safety
will provide safe and usable equipment’s for personal care and
safety of the participants with a disability. These products are consumable and home
modifications that will deliver comfort as well as a secure environment.
Assist- Travel/Transport
Productive care support provides travel and transport services for
people with a disability. Assistance with travel and transport include support that enable
participants to build capacity to independently travel, including through personal transport related aids and equipment, or training to use public transport
Assist-life stage, Transition
at productive care support we provide assistance with life stages
and transitions, including support coordination short and long term, this involves strengthening a
participant’s abilities to coordinate their support and participate in the community.
Participate Community
At Productive care support we can arrange social, community and
recreational activities that participant can take part in and engage in. we strongly believe that by
encouraging our participants in community activities can improve mental and physical wellbeing
as well as strengthen communication skills and confidence
